Brak Odo was killed by a skeleton with a gem in its forehead.
So was Cronuk

Steelf the dwarf died when the gem skelly reanimated the first time (mission not accomplished)
Jaggo died to his deceased zombie wife Ethel, by strangulation, a practice the two had enjoyed consensually in life
Vinrin the Halfling was strangled by Ethel the Zombie, non-consensually
Borgo dies to a disembodied zombie hand he took with him while running from the rest of the zombie
Darmek dies to a giant scorpion
Ushlynk died to the same one
Becca falls to a skeleton archer on the way to the barrowmaze
Asa dies to a skeleton archer and leaves Rex bereft
Cami died to a pit trap with spikes and yellow mold

Druidly was almost killed by bandits, but was saved by two golden tickets.
Gemma fell to 5 skeletons because she was alone. The party recovered her body, and Bancroft held a funeral service.
Hannah fell to a skeleton’s arrow, and drowned before El Viz could rescue her.
El Viz died to the skeleton’s next arrow.
Squinch followed them faithfully into the afterlife.
Kethry died to four giant toads. She will rest in peace, even though Wiz was careful not to catch her in his spell effect.

Tohru fell to a blow from a stone statue. Her duck survived.

Druidly is eaten by a pack of mimics.

Wyz, Alandor, and Perch died to an infernal book.
Vorn, Cade and Roderick died to carnivorous horseflies and robed skeletons.
Pippin, Thrain, and Caleb fell to lizard-man statues.
Riyou squeezed through a wall of rubble, fell into a pit, and was poisoned to death by giant scorpions. Her body lies somewhere the rest of the party cannot reach.
Pippin, Thrain, and Caleb fell to a gray ooze. Again. It turns out they had been dead since the statue room; nobody noticed, including them. This time it took.
Vrak the Scarred was finally paid in full – the gray ooze settled the company’s whole debt to him at once, by eating him. He died owed nothing, holding the light, reaching for a gem still stuck in the wall.
Morrigan went under a zombie’s fists the same night, which was an insult – she crossed every pit in the maze with a torch in her teeth and deserved a better monster. With both of them gone, the leftover Monday company was retired.
Irulan went down covering the retreat and never woke up. Her chainmail, her yew holy symbol and her elvish longsword all went to Bancroft, who did not want any of them on those terms.
Clarisse was lost in the dark during the same retreat and died sometime in the night. Nobody knows where. Her lamp is still burning down there somewhere.
Fantasia Heavy fell off a rope twenty feet from daylight, carrying the rope ladder up on her back so everyone else could climb out. She had one hit point for the whole ten days she was alive and she insisted, to the end, that she was a dragon.
